Mahayograj guggul, a compound containing the oleo gum resin of Balsamodendron mukul and other ingredients is one of the important anti-rheumatic compounds of Ayurveda. The components of Rumalqya tablets and cream have anti-inflammatory, anti-arthritic, anti-phlogistic, antispasmodic, analgesic, anti-infective, diuretic, tonic, cardiac and general stimulant, hypocholesterolaemic effect, increased uric acid excretion and prevention of osteoporosis and degeneration changes as quoted by Gujral et al 1960, Bhargava, 1962, Mahesh Rai et al 1967 and others. In the present series, the incidence of rheumatoid arthropathy was higher than that of osteoarthroses. The conditions were predominant in males of older age groups, shown by Bhattacharya et al 1974.
The legal difference between a cosmetic and a drug is determined by a product's intended use. Different laws and regulations apply to each type of product. Segment is usually selected on the basis of CXR. 20-60 ml of warm buffered saline is injected and gently aspirated. A return of 50-60% is expected in normal persons and lesser in diseased lung.
Suppliers should define plant extract quality for their customers and guarantee reproducibility of that quality from batch to batch. Last but not least, extracts should be easy to formulate within the desired food matrix and it should be possible to demonstrate their bioavailability and efficacy in the final product. Professor Jrgen Vormann, from the Institute for Prevention and Nutrition in Munich, Germany, spoke on the role of coenzyme Q10, an endogenous substance with many health benefits, including the maintenance of healthy skin function. Toxicity of Indigenous Drug Combination: In the present series there was no toxicity noticed in any of the patients receiving the indigenous drug combination. Aggarwal 1970 ; and Gogate 1957 ; also found no incidence of toxicity while using this indigenous drug combination Table 10 ; . From the foregoing observations it can be said that the indigenous drug, Rumalaya, has got a place in the management of chronic rheumatic disorders like rheumatoid arthritis, ankylosing spondylitis and osteoarthritis. Its advantage over the other anti-rheumatic drugs is that it is virtually non-toxic and as such the patients do not require constant medical check-ups, including laboratory investigations which are often time-consuming and expensive. By using Ruumalaya in these chronic rheumatic disorders the patients can be spared the hazards of commonly used anti-rheumatic drugs such as gastrointestinal complications, bone marrow depression, renal damage, hypertension, moonface, glycosuria etc. Even with all these undesirable side effects, the presently available antirheumatic drugs are still unsatisfactory in controlling the disease process completely. They merely offer some symptomatic relief. Moreover, some of the side effects produced by these drugs are more troublesome and harmful to the patients than the disease process itself. All the presently available anti-rheumatic drugs except salicylates and chloroquine are costly. This is also a major disadvantage in their prolonged use. In this respect also Rimalaya affords a welcome relief because the cost of Rumalayq is not very high. The disadvantages of Rumalaya are that it is slow in producing its effect i.e. it takes about three weeks time to produce its therapeutic effect. Even so, with side effects non-existent, it has a definite place in the treatment of these commonly occurring chronic rheumatic disorders. As the number of cases in the present series is small, it is felt that a trial on a larger series should be taken up for any definite final conclusion. SUMMARY AND CONCLUSION 1. A crossover clinical trial on rheumatoid arthritis, ankylosing spondylitis and osteoarthritis was carried on with an indigenous drug combination Rumalaya ; amongst 50 consecutive cases admitted to the hospital. It included 29 cases of rheumatoid arthritis Group I ; , 13 cases of ankylosing spondylitis Group II ; and 8 cases of osteoarthritis Group III and benemid.
Where the Vm Km is constant term and the whole equation now looks like that for first order elimination, with Vm Km a pseudo first order elimination rate constant, k'. Therefore at low plasma concentrations we would expect first order kinetics. Remember, this is the usual situation for most drugs. INTRODUCTION Arthritis is one of the most common medical problem affecting joints of the knees, hips, spine and hands. The etiology of osteoarthrosis is not clear and differs according to the age, sex, genetic factors and obesity. Repeated trauma is known to be associated with a higher incidence of osteoarthrosis. The common symptoms of osteoarthrosis include pain in the joint during or after use, discomfort in the joint before or during a change in the weather, swelling and stiffness in the joint particularly after using it and bony lumps on the middle or distal end of the joints of the fingers or base of the thumb. There may be associated loss of flexibility of a joint. The acute pain of early osteoarthrosis often tends to fade within a year of its appearance, but may revert back with overuse of the affected joint. No single test can diagnose osteoarthrosis as diagnosis is based on a combination of clinical history, physical examination, radiological examination of the joint and if required aspiration of synovial fluid for confirmatory diagnosis. Treatment options available for osteoarthrosis include topical systemic pain relievers, NSAID's, COX-II inhibitors and intra-articular injections of corticosteroids or hyaluronic acid. NSAIDs like aspirin, ibuprofen, ketoprofen and naproxen sodium are widely used pain relievers, but are not effective in preventing the progress of the disease. All NSAIDs have the risk of side effects that increases when used at high doses for long-term treatment. Intra-articular injections with a corticosteroid or hyaluronic acid offer pain relief for 4-6 months but are associated with increased chances of intra-articular infections. Rumalaya forte is a polyherbal formulation containing extracts of Boswellia serrata, Alpinia galanga, Commiphora wightii, Glycyrrhiza glabra, Tinospora cordifolia and Tribulus terrestris. These ingredients have shown to have anti-inflammatory, anti-arthritic, immunomodulatory, muscle relaxant and analgesic activities. Hence, the present study was conducted to compare the efficacy of Rumalaya forte in terms of relieving pain, inflammation and improving free mobility of the joint and safety in patients suffering with osteoarthrosis. METHODOLOGY Study population A total of 57 patients attending the orthopedics outpatient department of Grant Medical College and Sir J.J. Group of Hospitals, Mumbai, diagnosed with osteoarthrosis of the knee were enrolled after obtaining informed written consent and antiox.

This study was undertaken because even after a course of specific therapy with penicillin the polyarthralgia persisted in a large percentage of cases of secondary syphilis. Hence it was decided to give a therapeutic trial to a combination of oral Rumalaya tablets along with the usual parenteral penicillin. This combination was tried in 20 consecutive patients of secondary syphilis with polyarthralgia. The dosage of Rumalaya tablets was one tablet 3 times a day. The majority of the patients had treatment for eight days duration. There were 20 patients of secondary syphilis with polyarthralgia under study, out of which 7 were females and 13 were males. Out of the 20 patients treated with a combination of oral Rumalaya tablets and penicillin therapy, 15 patients showed marked improvement, 3 showed only slight improvement and 2 cases showed no improvement. At the end of treatment 75% did not complain of any joint pains in the series under study. These results with the combination of Rumalaya tablets and penicillin compare very favourably with the author's previous experience with penicillin alone and clavamox.

Mahayograj guggul has anti-inflammatory and anti-arthritic activity. The most important advantage of this drug is its low toxicity and hence it can be administered for a long time. It has been reported to be useful by Aggarwal, 1970; Gogate, 1957; Bhattacharya, 1974; Tejeswar Rao, 1974 and B.V. Mehta & Kochar, 1972. This oleo-gum resin is considered a sovereign remedy in ancient medicine. It is quickly absorbed and is excreted by the skin, mucous membranes and kidneys and in the course of its excretion disinfects this secretions, improves the functions and stimulates the activity of the respective organs. It has marked antiseptic properties also. Apart from anti-inflammatory action it has anti-phlogistic, anti-arthritic, anti-spasmodic, analgesic and tonic effect. It stimulates uric acid excretion and prevents osteoporosis and degenerative changes. These reported studies prompted us to try the clinical results of the indigenous drug Rumalaya tablets. MATERIAL AND METHODS In a controlled study of 75 cases of osteoarthritis at the Rheumatology Clinic of Ramkrishna Mission Seva Pratisthan, 50 cases were treated with Rumalaya tablets 2 tablets t.i.d. daily after meals for a period of nine months. Twenty five similar cases served as controls; they were put on other drugs and were not given Rumalaya. They were reviewed by the same doctor using the same criteria once every month and the detailed findings, subjective and objective response and other effects, if any, were recorded very carefully for the final assessment and evaluation. Routine urine testing, haemogram, E.S.R. estimation and occult blood test in stools were carried out regularly. Gastric intolerance or any other side effects were also looked for and noted. OBSERVATIONS The ages in both the groups varied from 23 to 82 years, the average being 52 years. Females predominated slightly being 52%, the males being 48%. Average haemoglobin was 75% and E.S.R. 8 mg at the end of one hour. Occult blood was absent in stools and routine urine examination did not reveal any abnormality. After confirming the diagnosis clinically by laboratory, x-rays and other findings, the patients were given Rumalaya tablets 2 t.i.d. after meals. All the examinations, investigations and x-rays were carried out by the same person initially and at each follow-up for 9 months. The initial findings in all the cases of both the study and control groups were: pain in 50 100% ; and 25 100% ; cases in respective affected joints, about 45 90% ; and 23 92% ; showed crepitation on movement in the affected knees, 17 34% ; and 8 32% ; had stiffness in the affected joints and 10 20% ; and 2 8% ; of these had restriction of movements of their joints. RESULTS In the study group on Rumalaya 20 out of 50 patients i.e., 40% of the patients did not have any pain at the end of the trial i.e., pain completely disappeared on therapy, other 20 i.e., 40% did feel less pain and felt relieved to some extent and continued to remain much better with increased movement while they were on Rumalaya. The remaining 10 patients i.e., 20% did not have any appreciable change of any kind and remained status quo. None of the patients got worse or had any gastric irritation, intolerance or untoward effect during all these nine months. There were no undue effects on E.S.R., haemogram remained static, x-rays showed some progress towards improvement. Five patients 10% ; did not have any physiotherapy. The remaining patients received either short-wave.
Results Sixty primary prostate tumor specimens were analyzed for activation of MAP kinase: 20 from transurethral resections and 40 from radical prostatectomies. At sampling, only five patients had received prior therapy: four patients received radiation treatment range, 5 days to 9 years prior to sampling ; , and one patient had an orchiectomy 4 months prior ; . Adjacent nonneoplastic prostatic epithelium showed little or no staining with activated MAP kinase antiserum; however, virtually all of these samples displayed some level of activated MAP kinase immunoreactivity in a fraction of the glandular epithelial cells Fig. 1A ; . In general, basal cells showed activated MAP kinase staining more frequently than secretory cells, and at times, the basal cell layer showed a nearly continuous staining pattern. Activated MAP kinase was frequently detected in the surrounding smooth muscle stroma, endothelium, and peripheral nerves. Lymphocytes were virtually always negative with only the rare cell staining for activated MAP kinase. In most cells positive for MAP kinase immunoreactivity, both intense nuclear and more diffuse cytoplasmic staining was apparent. In neoplastic prostatic epithelium, the level of activated MAP kinase varied, ranging from as little as no detectable activated MAP kinase to more than 50% of tumor cells displaying activated MAP kinase. For purposes of statistical analysis, the tumor specimens were grouped into three categories based on the percentage of tumor cell nuclei staining with the phospho-MAP kinase antibody: zero, low 10% tumor cells positive ; , and high 10% tumor cells positive ; . This scoring system was highly reproducible and virtually bimodal and geriforte.

Local spread was modelled and found to occur at a rate of about 2 km per day or approximately 15 km per week and equals the flight distances covered by Culicoides. Wind may affect spread over long distances. In particular, the density of the observed wind events contributed, at least in part, to explaining the spread of BTV. In conclusion, changes in climatic conditions coupled with increased worldwide traffic might increase the risk in the appearance and the establishment of diseases in parts of Europe that were thus far exotic to those regions. This presentation will describe and compare the toxic effects of lead Pb ; on cell and tissue culture systems, i.e., in vitro models, which represent the constituents of the central nervous system CNS ; . Studies from our laboratory have identified differential effects of Pb on neurons and astrocytes, providing insights into neuronalglial interactions in the response of the CNS to toxicants. Astrocytes are endowed with several antioxidant systems that may protect them from metals and other oxidants. Astrocytes also secrete interleukin-6 IL-6 ; during oxidative stress, which may protect neurons. Furthermore, astrocytes accumulate much more Pb than do neurons, a phenomenon with many implications for neuronal stress responses. A protein of interest in understanding the differential glial response is the 78 kDa glucose-regulated protein GRP78 ; , an endoplasmic reticulum ER ; molecular chaperone involved in protein folding, assembly and trafficking. This protein chaperones IL-6 secretion by astrocytes. GRP78 also plays a role in protection against cytotoxicity and apoptosis induced by environmental insults. Work from our laboratory demonstrates that Pb directly binds to GRP78 protein and increases GRP78 levels in astrocytes. Furthermore, Pb induces the aggregation of a GRP78-EGFP fusion protein in astroglial cells, in marked contrast to its normally diffuse cytosolic distribution. GRP78 depletion significantly increases the sensitivity of glia to Pb exposure as indicated by the generation of reactive oxygen species ROS ; . Binding of Pb to GRP78 is also consistent with the observations that Pb co-migrates with high molecular weight 66kD ; protein fractions. These data indicate that Pb directly targets GRP78 and induces its compartmentalized redistribution in living cells and that GRP78 plays a protective role in Pb neurotoxicity.
Marketing troglitazone in the UK, wrote to doctors and pharmacists warning about these reactions and providing advice about monitoring liver function tests. Medicines Control Agency have now become aware of over 130 cases 6 fatal ; worldwide of hepatic reactions to troglitazone. The average time to the onset of the reaction was 3 months. In the UK, some 5, 000 patients have been treated, with one reported case of liver dysfunction. At present no clear risk factors for the development of hepatic reactions have been identified which might allow the drug to be used safely in some patients. The frequency of these reactions is currently unclear. Approximately 370, 000 patients worldwide have been treated with the drug for at least 3 months. Overall it is considered that, based on present information, the risks of troglitazone therapy outweigh the potential benefits. It has therefore been voluntarily withdrawn from the UK as from 1 December 1997 by the companies concerned, who have informed doctors and pharmacists by letter. Fenfluramine and dexfenfluramine withdrawn In the last issue of Current Problems in Pharmacovigilance reported on the occurrence of valvular heart disease associated with the combination of fenfluramine and phentermine. Following publication of a paper in the New England Journal of Medicine the number of cases reported in the USA increased to more than 100. Some of these were associated with dexfenfluramine or fenfluramine treatment alone but none with phentermine alone. Patients developed mitral, aortic, tricuspid or mixed value disease. A survey in the USA also described asymptomatic cases where only physical examination and or echocardiography revealed the abnormalities. The manufacturers have voluntarily withdrawn fenfluramine and dexfenfluramine worldwide and effectively these drugs have not been available in the UK since 1 October 1997. Doctors and pharmacists were informed of this via an Epinet message and by a and l-tryptophan and Order rumalaya.
